Copyright Infringement Notification

If you believe that your work has been copied and posted on our website in a way that constitutes copyright infringement, we urge you to send us a notification. This notice should provide a detailed description of the copyrighted work you claim has been infringed, as well as the URL or other specific location on our site where the material can be found. In order to act on your complaint, we will also require your contact information.

Procedure for Reporting Copyright Infringements

Notifications of claimed copyright infringement should be directed to our designated agent at dmca@leaseterminationform.com. To ensure a swift and effective response, your notification must include the following:

  • A physical or electronic signature of a person authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
  • Identification of the copyrighted work claimed to have been infringed, or, if multiple copyrighted works at a single online site are covered by a single notification, a representative list of such works at that site.
  • Identification of the material that is claimed to be infringing or to be the subject of infringing activity and that is to be removed or access to which is to be disabled, and information reasonably sufficient to permit the service provider to locate the material.
  • Information reasonably sufficient to permit the service provider to contact the complaining party, such as an address, telephone number, and, if available, an electronic mail address at which the complaining party may be contacted.

We will review all properly submitted copyright infringement notifications and take whatever action we deem appropriate, including the removal of the challenged material from our website.

Counter-Notification Procedures

If you have received a notification of alleged infringement regarding material you have posted on our website and believe this to be in error, you may submit a counter-notification. This must be sent to our designated agent at dmca@leaseterminationform.com and include:

  • Your physical or electronic signature.
  • A description of the material that has been removed or to which access has been disabled and the location at which the material appeared before it was removed or access to it was disabled.
  • A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification of the material to be removed or disabled.
  • Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the federal court for the judicial district in which the address is located, or if your address is outside of the United States, for any judicial district in which the service provider may be found, and that you will accept service of process from the person who provided notification of the alleged infringement.

Upon receipt of a valid counter-notification, Lease Termination Form will promptly provide the complainant with a copy of the counter-notification, and inform them that we will replace the removed material or cease disabling access to it within 10 business days. Unless our designated agent first receives notice from the complainant that they have filed an action seeking a court order against the content provider, we will replace the removed material and cease disabling access to it.
